Palliative, Hospice and End of Life Care
At Greater Health Disability Services, we provide compassionate, holistic, and person-centred palliative, hospice, and end-of-life care services designed to support individuals living with life-limiting, chronic, degenerative, or terminal illnesses.
Our goal is to enhance comfort, preserve dignity, promote quality of life, and provide emotional reassurance to participants and their families during every stage of care. We understand that each person’s journey is unique, which is why our services are tailored to individual clinical needs, cultural preferences, personal wishes, and family goals.
We work collaboratively with hospitals, general practitioners, palliative care teams, social workers, allied health professionals, and families to ensure continuity of care and a safe transition into the home or preferred care environment.
