Support Coordination
At Greater Health Disability Services, our Support Coordination service helps NDIS participants understand, implement and make the most of their NDIS plans. Our goal is to empower participants to build the skills and confidence needed to navigate the NDIS and access the supports that best meet their needs. Support Coordinators work closely with participants, families and carers to ensure supports are connected, coordinated and aligned with the participant’s goals. We focus on promoting choice, control and independence, while ensuring participants receive high-quality services that help them live the life they choose.
What Support Coordination Involves
Our Support Coordinators assist participants to:
Understand their NDIS plan and how funding can be used
Connect with suitable service providers and community supports
Coordinate services to ensure supports work well together
Build capacity to manage and coordinate supports independently
Navigate service agreements and provider options
Address barriers that may impact access to supports
Prepare for plan reviews and monitor progress toward goals
Our Approach
At Greater Health Disability Services, we take a person-centred approach to Support Coordination. This means every participant is supported based on their unique goals, preferences and circumstances. We work collaboratively with participants to ensure they remain at the centre of all decision-making. Our coordinators focus on building long-term skills so participants can increase independence and strengthen their support networks over time.
Who Can Access Support Coordination
Support Coordination is available to NDIS participants whose plans include funding for this service. The level of support provided will depend on the type of Support Coordination included in the participant’s plan.
These include:
Support Connection – short-term support to connect participants with providers and community supports.
Support Coordination – ongoing assistance to coordinate services and build capacity to manage supports.
